Caring for You
An interdisciplinary team of Casa de la Luz professionals and volunteers provides compassionate end-of-life care to you and your loved ones. This team forms a community of care, with you and your loved ones at its center. Casa team members meet with you and your family and make scheduled visits according to your needs. Hospice services are available 24 hours a day, 7 days a week.
The team develops a plan of care focused on your goals, well-being, and needs for pain management and symptom control. The team members coordinate care, manage the patient’s pain and/or symptoms, provide nursing and personal care, and coach your family and caregivers in your care. The social services team is available to assist you and your loved ones with the emotional and spiritual aspects of living with dying. A counselor is available to provide grief support to your family. Other individuals can be brought in to provide special services like speech and physical therapy when appropriate.

- Determines if an individual is medically eligible for hospice care
- Directs and approves care with hospice interdisciplinary team
- Available to assess patient needs and manage care of symptoms

Social Worker
- Assesses social, emotional, and financial needs
- Counsels and supports patient and family, often in collaboration with chaplain
- Assists in mortuary planning and serves as link to other community resources, as needed
- Provides initial bereavement support

Bereavement Counselor
- Provides individual counseling for patients and family members
- Leads grief support groups
- Follows a patient’s loved ones for 13 months after the patient’s death

RN Case Manager
- Develops and implements plan of care
- Educates and supports patient and caregivers
- Assesses patient needs and provides physical care, including symptom control and pain management

Volunteer
- Gives support to patient through companionship
- Offers respite for the family caregiver
- Assists with other practical, non-medical services
- Some volunteers lend professional expertise through the hospice’s complementary alternative therapies program

Therapist & Dietitian
- Directs therapy for symptom control or purpose of maintaining basic functional skills
- Provides services for speech and/or swallowing problems
- Offers nutrition and dietary counseling and education

Home Health Aide
- He/she is a certified nursing assistant
- Provides personal care, including bathing and grooming assistance
- Educates patient and caregivers about comfort care and physical safety

Chaplain
- Assesses spiritual needs of patient and family
- Provides spiritual counseling or referral to community clergy, with respect to individual’s faith traditions and spiritual beliefs
- Offers memorial and funeral preparation assistance, including help with planning or officiating at memorial services

- Assesses and responds to patient needs after office hours
- Nurses, social worker, and chaplain available
