An added layer of support
Working in collaboration with your primary and/or specialized providers(s), an interdisciplinary team of Casa de la Luz Palliative Care professionals provide recommendations for symptom management and support for addressing psychosocial and spiritual needs.
The Palliative Care Nurse Practitioner and Licensed Clinical Social Worker develops a plan of care focused on your goals, well-being, and needs for pain management and symptom control. Other individuals, including volunteers, can be brought in to provide special services like music therapy when appropriate. The Casa de la Luz Palliative Care team members maintain regular communication with your attending physician, augmenting the holistic care both you and your loved ones receive.
