Making a Referral for Hospice Care

Any individual can call Casa de la Luz Hospice to request a hospice care evaluation. Hospice care helps care for terminally ill individuals with six months or less to live who have decided to stop curative treatments and focus on comfort care.

If you know someone who fits this requirement, hospice care might be the right option. The hospice would be able to provide comfort care to the patient, and support to the caregiver(s). Patients are most often cared for in their own home, and hospice team members visit regularly to manage and guide care.

Individuals often choose to discuss the option of hospice care with their primary care practitioner, but it’s not a requirement. Any individual can call a hospice and request information about services or a hospice evaluation by a representative.

Take charge of your healthcare needs, and learn about your options. Hospice care can be a difficult topic to discuss, but our team members can answer your questions regarding services and the end-of-life process. Requesting information or a hospice evaluation does not mean a person has to commit to signing up for care.
