The first misconception is that caregivers remain with the patient 24 hours a day, 7 days a week. In fact, the 24-hour care of hospice only means that support from the hospice is available all the time. Hospice patients must have a primary caregiver, a family member or a hired caregiver, in the home. The hospice offers patients a team of individuals who visit intermittently, based on patient need, and then should needs arise outside of regularly scheduled visits, the patient or the caregiver can call the hospice for support.

We have a designated after-hours team that is available to handle patient needs that arise between the hours of 5 p.m. and 8 a.m. We know that your healthcare needs don’t happen on a schedule, and that’s why staff is available to assist you whenever you need it. Our after-hours team is comprised of nurses, a social worker, and a chaplain. They are all experts in hospice care, and will provide you with the compassionate and professional care you need.

Our after-hours coordinator will be happy to put you in contact with the appropriate team member. An after-hours team member will talk to you on the phone to evaluate your needs, and if a visit to the home is necessary, the appropriate person will make a visit.
